Integrated Care Teams

Each of the five CCG localities has a multi-disciplinary Integrated Care Team of healthcare and social professionals working together to keep people happy, healthy and at home. By working with multiple organisations, we aim to arrange the right support and care, particularly where there are complex needs. The ICT can help reduce the risk of hospital admission and speed up the safe discharge of those in hospital, when ready to return home. This is achieved by

  • Giving GPs and other medical professionals access to a team of experts when making complex care decisions
  • Making or supporting referrals, reducing the time spent by professionals on setting these up
  • Tracking progress and following up on patients referred to the ICT, freeing up professionals
  • Keeping the patients’ GP informed of progress, outcomes and when the patient is no longer under active ICT care

The ICTs are at varying stages of development and the focus recently has been to work with colleagues across organisations to have a well-functioning, integrated team that is supported and dedicated to improving out of hospital care.  Salus Paramedic Practitioners are now co-located and work closely with the ICT staff.

Recent developments across all localities include the introduction of Community based Frailty MDTs to prevent/reduce the risk of hospital admission, and the introduction of Locality Access Points to support referrals between services. These new services will also make use of the ICT's shared knowledge base and skills.
