This North East Hampshire and Farnham CCG Vanguard project, provides paramedic practitioner support in Fleet, Farnborough, Aldershot and Yateley. Paramedic practitioners are assigned to GP practices in support of the home visiting service.

GPs continue to triage all home visit requests but the additional resource has a number of advantages for patients:

The Paramedic can spend longer assessing a patient, and can visit earlier in the day

The direct relationship between the Paramedic and GP gives enhanced clinical support

The Paramedic has full access to a patient’s medical records at the time of visiting
